Welcome to Diabetes Forward

Whether you are establishing care with an endocrinologist for the first time or continuing your care with Dr. Benjamin, we are delighted to welcome you to Diabetes Forward. Our goal is to make your transition into the practice as seamless and straightforward as possible.

Your initial visit is focused on understanding your full medical picture and making meaningful, data-informed decisions. This may include:

Review of lab results and CGM data (if applicable)

Before Your Visit

To make your visit as productive as possible, please come prepared with the following:

  • A complete and up-to-date list of all medications, including insulin, oral medications, supplements, and doses
  • Recent lab work, imaging reports, or bone density (DEXA) scan results, ideally within the past 3–6 months (if available)
  • Access to your CGM data or glucose records (if applicable)

If you have not had recent lab work, we are happy to help arrange this prior to your visit. We are also pleased to help connect your cloud-based CGM and insulin pump data to the clinic in advance of your appointment.

Having this information available is essential to making the most of your visit.

CGM

Continuous Glucose Monitoring (CGM)

If you are using a continuous glucose monitor, you will be asked to share or upload your data prior to your visit. If you are not currently using CGM, we can discuss whether it may be appropriate for you. Coverage is often dependent on insurance criteria.

After Your Visit

After Your Visit

Care does not end at the appointment. Depending on your needs, follow-up may include:

  • Scheduled follow-up visits
  • Ongoing review of glucose data
  • Adjustments to your treatment plan over time

Insurance & Payment

Insurance & Payment

We accept most major insurance plans. Coverage for services such as CGM may vary depending on your plan.
Additional care options that provide more frequent support between visits may be available and can be discussed during your visit.
